This medication diary is especially useful for people that are starting new medication and need to ramp up the dosage until they reach the desired amount prescribed by their doctor.

 There is enough space to write down daily dosage and side effects.  E.g. 6ml morning – very sleepy most of the day.

How to use the medication diary notebook

1. Write your medicine at the top of the page. 
2. If you need to ramp up the dose, you can add the start and target dose.
3. Add any notes in the provided box
4. Track your medicine over 31 days. You can use the space provided to list your dose increases, side effects and results.

The medication diary notebook comes as a single 40 page notebook.

One per page will track a month worth of new medicine.  

This means the 40 page notebook will track 40 different medicine changes or one medicine for over 3 years.

The inside front cover has a start and end date of the book and room for two doctors / specialists or chemist details in case you need to contact the health professional about side effects.

The inside back cover has a summary to help find the specific medication that you have been tracking to show your doctor.

Size and materials used

Our medication diary notebooks are handmade and come in B6 size (17.6 cm x 12.5 cm / 6.9″ x 4.9″ ) Which is slightly smaller than A5 / half US letter. 

All our notebooks have a 200 gsm thick card stock cover and are binded with two staples. The inside pages are professionally printed on 100 gsm paper which is super smooth to write on and bright white in colour.
